The Hurlbut Nursing Home, a 160 bed Skilled Nursing and Rehabilitation facility located in Brighton, NY, is seeking a highly motivated and compassionate Social Worker to join our team - Part Time Days ( 24 hours a week ) as a Social Work Assistant.

The ideal candidate will have knowledge of Medicaid and Medicare insurance and be able to work closely with residents and their families. As a Social Worker, you will be responsible for assisting with admissions and discharges, as well as helping with MDS.

Responsibilities:

Coordinate with the admissions team to ensure that residents are admitted to The Hurlbut in a timely and efficient manner
Ensure admission agreements are completely timely
Help residents and their families understand the benefits and limitations of Medicaid and Medicare insurance
Work closely with the nursing staff to identify and address the social and emotional needs of residents
Assist with discharge planning, including arranging for home health services or other necessary services
Conduct social assessments and develop care plans based on the needs and preferences of each resident
Provide emotional support and counseling to residents and their families
Help residents participate in recreational activities and social events
Coordinate with community organizations to provide additional support to residents

Requirements:

Preferred Bachelor's degree or related field
Knowledge of Medicaid and Medicare insurance
Ability to work closely with residents and their families
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Familiarity with MDS
Compassionate and caring personality
