About Job

RESPONSIBILITIES

  • Perform all enrollment activities as required by the Department of Health (DOH) and the Lead Health Home requirements. This includes conducting outreach contacts, obtaining necessary consents, and assisting in gathering supporting documentation.
  • Conduct comprehensive assessments and take responsibility for creating and managing the member's Plan of Care (POC).
  • Coordinate care and services among providers and community resources. Assist in coordinating services and benefits in case of hospitalization or admission to any facility/residence. Facilitate discharge from any hospital/facility and help with transfers to another Care Management Agency (CMA) or program if necessary for continuity of care.
  • Coordinate and conduct Interdisciplinary Team Meetings to facilitate the creation of the Plan of Care and coordination of services.
  • Assist the member in obtaining and maintaining public benefits, including but not limited to Medicaid, Cash Assistance Eligibility, Social Security, Supplemental Nutrition Assistance Program, Housing, Legal Services, and Employment.
  • Educate the member, caregiver, and supports on the member's diagnosis or condition, as well as their needs, strengths, and goals.
  • Act as an advocate for the member in various institutional settings, within the home or community, while interacting with different providers, or when connecting the member with additional services or resources.
  • Make referrals to services and additional resources, assisting the member in following through with services or appointments.
  • Meet all monthly billing requirements and documentation as stipulated by DOH and Lead Health Home. Perform home and field visits, as required.
  • Maintain accurate case records and document all services.
  • Attend regular supervision sessions with a direct supervisor.
  • Perform additional duties as assigned.

REQUIREMENTS

  • Must be open to working 40 hours per week, as this is a full-time position.
  • Must be willing to travel within ONE of the Four Boroughs-Bronx, Manhattan, Queens or Brooklyn
  • Minimum requirement: Bachelor's Degree
  • Proficient in modern communication, including but not limited to the usage of a computer, Office 365, and Healthcare Management systems.
  • Must speak and write Haitian Creole

Job Type: Full-time

Pay: $26.00 per hour
