Overview
This position will conduct clinical assessments, facilitate family team conferences, develop service plans, make referrals to needed treatment, supervise case management staff, and ensure outcomes and benchmarks are achieved.
Responsibilities
- Directly supervise and provide clinical oversight to case management line staff. To ensure that case management staff are addressing clients' needs, taking appropriate actions, and documenting activities appropriately.
- Weekly team and individual supervision meetings.
- Case/chart reviews.
- Case management and home visits are timely and properly documented.
- Conduct clinical family assessments, create service plans, and make referrals to needed treatment. Couples referred to SHR services need to be assessed at the outset to ensure that appropriate treatment referrals and other needed services are identified at enrollment.
- Weekly meetings with staff and training partners.
- Referrals to needed treatment.
- Review client charts and records entered in tracking system. To ensure that all program activities are properly documented and appropriate actions taken.
- Generate weekly reports.
- Home visit and other contacts are made, as required.
- Couples referred to treatment are attending therapy, as required.
- Conduct in-service clinical training to Case Management staff. Case Management staff will need additional ongoing training to address clinical issues and needs of clients; specifically on risk assessment.
- Improved understanding of clinical issues by Case Management staff.
- Facilitate Couples Team Conferences for Couples enrolled in program. To discuss treatment plan for couples, to provide update on status of treatment, and to collectively develop a plan of action for the family.
- Couples Team Conferences occur and are documented in the tracking system(s).
- Provide clinical supervision to limited permit psychologist and psychology interns. To ensure that clients' needs are met and that the appropriate level of clinical actions occur and are documented.
- Improved understanding of clinical issues by clinicians.
- Generate weekly reports.
- PhD Required.
- 1-3 years' experience conducting assessments required.
- Provide clinical supervision and training to case management staff (High proficiency).
- Managerial skills demonstrated in a start-up program (High proficiency).
- Excellent oral/written communication skills (High proficiency).
- Familiarity with Microsoft Word, Excel, and Access (Medium proficiency).
- Flexibility and ability to find creative solutions to challenges (High proficiency).
- Psychologist Required.
- OHS Annual Assessment Required.
