**HYBRID SCHEDULE OPPORTUNITIES**


Job Summary:

Mountain Park Health Center is actively seeking Mental Health Providers (Team made up of Licensed Clinical Psychologists, Licensed Clinical Social Workers & Licensed Professional Counselors) to join our well-established and accredited team of providers. As a Federally Qualified Health Center (FQHC) in Arizona, our mission is to sustain and improve health by providing excellent, patient-centered primary care.


As a Mental Health Provider, you will provide traditional counseling services to Mountain Park patients aged 6 years through adulthood. Your work will help diverse populations access high-quality care and ensure culturally competent services. Our integrated health model ensures comprehensive services for our patients, fostering collaboration between medical and behavioral health professionals.


ESSENTIAL FUNCTIONS:

  • Provides evidence-based outpatient psychotherapy, including but not limited to: Acceptance and Commitment Therapy, Cognitive Behavioral Therapy, Motivational Interviewing, and Solution-Focused Therapy.
  • Conducts individual, family, and couples therapy sessions.
  • Performs intake assessments, develops treatment plans, and updates them with measurable, age-appropriate objectives.
  • Provides assessment and DSM-5TR diagnostics for patients referred by medical staff.
  • Collaborates and co-manages behavioral health care alongside medical providers and behavioral health consultants.
  • Delivers care in-person and via telehealth as needed.
  • Maintains accurate clinical records, ensuring treatment and discharge plans align with compliance standards.
  • Offers crisis triage and intervention as needed.
  • Maintains regular and predictable attendance in accordance with organizational standards.
  • Travels to multiple locations during shift and within a reasonable amount of time.
  • Performs other duties as assigned to support patient care.

Minimum Qualifications:

  • Current licensure in the state of Arizona as a Clinical Psychologist, Clinical Social Worker or Professional Counselor.
  • Must be eligible for credentialing and contracting with all insurers.
  • Valid CPR Card

Preferred Qualifications:

  • Bilingual in Spanish/ English.
  • Training in Trauma-Informed Care.
  • Experience working within a multidisciplinary, integrated care model.
