Job Title: Case Worker
Pay Rate: $41–$43/hour (W-2)
Location: New York, NY 10016
Schedule: Monday–Friday | 9:00 AM – 5:00 PM EST
Hours: 8 hours/day | 40 hours/week
Department: Social Work – Non-Behavioral Health

Position Summary:

The Case Worker is responsible for providing comprehensive social services support, including assessments, referrals, and ongoing case management. This role involves conducting home visits, verifying client information, and ensuring individuals receive appropriate health-related and social services.

Key Responsibilities:

  • Conduct client interviews, complete initial assessments, and make appropriate referrals.
  • Manage and monitor cases of social service recipients.
  • Perform home visits to assess client needs and develop service plans.
  • Investigate, verify, and document information provided by clients.
  • Coordinate and deliver health-related and social services to clients.
  • Maintain accurate case records and comply with organizational standards.

Required Skills & Qualifications:

  • Minimum 3 years of relevant social services experience.
  • Bachelor’s Degree required (Social Work or related field preferred).
  • Strong knowledge of social service programs and community resources.
  • Excellent communication, assessment, and case management skills.
  • Ability to work independently and manage multiple cases effectively.

Job Types: Contract, Temporary

Pay: $40.00 - $43.00 per hour

Expected hours: 40 per week

  • Bachelor's (Preferred)

Experience:

  • Case Worker: 3 years (Required)
  • SOCIAL WORK: 3 years (Preferred)
