Sentara RMH Medical Center is now hiring an Medical Social Worker. This is a position that will be responsible for the provision of medical social work services to patients.EducationMaster's Degree - Social WorkCertification/Licensure
  • Basic Life Support (BLS) - Certification - American Heart Association (AHA) RQI within 90 days of hire
  • Certified Case Manager (CCM) - Certification - Commission For Case Manager Certification (CCMC) within 1 year of hire
Experience
  • 1 year of Case Management or other related experience
Duties/Responsibilities
  • Responsible for the provision of medical social work services to patients receiving care in the hospital setting.
  • Assesses the social, psychological, cultural, environmental, and financial situations, as well as disposition needs for each referred patient.
  • Collaborates with patients, families, healthcare team members, and community organizations, and using strong advocacy skills identifies needs and resources in the hospital and community.
  • Develop and evaluates effective transition plans taking into consideration the client diagnosis, prognosis course of treatment, past and present services, short-term and long-term goals, provider options, and/or available health care benefits as appropriate to the continuum of care.
We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay rate for Full Time employment is:$53,248.00-$88,753.60. Additional compensation may be available for this role such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
