About Job

Responsibilities:

  • Oversees primary needs of patient, including assessment of client and family/caregivers, and develops comprehensive plan of care.
  • Operate EHR/EMR Electronic Healthcare Records – Electronic Medical Record Software
  • Facilitate Video or Phone Therapy Sessions with clients
  • Must be organized and thorough with progress notes and plans of action
  • Provides Psychosocial Therapy
  • Monitors patient care through assessments, evaluations and/or patient records. Advocates for resources and removal of barriers.
  • Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.

What You Will Need

Licenses/Certifications/Registration:

  • Required Credential(s):
  • One or more of the following:
  • Counselor credentialed from the Michigan Board of Counseling obtained prior to hire date or job transfer date.
  • Social Worker credentialed from the Michigan Board of Social Work obtained prior to hire date or job transfer date.
  • Bilingual is a plus but not required
  • Preferred Credential(s):
  • (CAADC) Certified Advanced Addiction & Drug Counselor is a plus but not required
  • Substance Use Disorders (SUD) Counselor Certification is a plus but not required

Education:

  • Master’s degree of Social Work

Work Experience:

  • 0-5 Years

Job Type: Contract

Pay: $30.00 - $45.00 per hour
