Centennial Healthcare and Rehabilitation Center a skilled nursing facility located in West Philadelphia PA is currently looking for an energetic, approachable professional full time Social Worker to join our team of healthcare professionals.

Responsibilities include but are not limited to:

• Develop preliminary/comprehensive assessments for each resident.
• Report to direct supervisor as well as Director of Nursing Services.
• Encourage the resident/family to participate in the development and review of resident’s plan of care.
• Develop a written plan of care for each resident that identifies the social problems as well as the needs of the resident and set goals to be accomplished for each problem/need identified.
• Ensure that the social services components of the MDS reports are completed and signed in an efficient manner.
• Review nurse’s notes to determine if the care plan in place is being followed.

Qualifications:

• 1 year of social work nursing home experience
• BSW or related degree
• Excellent written and communication skills

Pay: $65,657.06 - $79,070.88 per year
