SERVICE & CARE COORDINATOR – Oahu (Iwilei Center – Kauhale Housing)

Do you excel at connecting people with care and services—and want to make a real impact? Mental Health Kokua (MHK) is hiring a Service & Care Coordinator for our Kauhale supportive housing site at Iwilei Center in Honolulu, Oahu. In this vital role, you’ll support adults living with mental illness by coordinating wellness services, appointments, and essential resources that foster stability and recovery.

This is a facility-based, client-centered position ideal for those with experience in behavioral health, care coordination, or housing services looking to contribute to Hawaii’s mental health and housing efforts.

Coordinator Responsibilities:

  • Care Coordination: Schedule and track medical, psychiatric, and behavioral health appointments; arrange transportation and ensure follow-through.
  • Client Advocacy & Support: Conduct intakes, assist with benefits enrollment, and help residents navigate social services and systems.
  • Community Building: Foster engagement through daily check-ins, wellness activities, and group programming that supports recovery.
  • Safety & Crisis Response: Monitor well-being, respond to emergencies using trauma-informed strategies, and uphold a calm, respectful environment.
  • Collaboration: Work closely with Kauhale staff, case managers, peer specialists, and providers to deliver wraparound support.
  • Data & Documentation: Maintain accurate, confidential records in HMIS, NextStep, and internal systems; upload key documents and progress notes.

Coordinator Qualifications:

  • Minimum: High school diploma or equivalent, plus 2 years in behavioral health, housing, or social services; OR Associate’s degree with 1 year experience
  • Preferred: Bachelor’s in Social Work, Psychology, Human Services, or related field; experience with HMIS or care coordination platforms
  • Current TB clearance, CPR, and First Aid certification (or ability to obtain)
  • Must pass a criminal background check and drug screening
  • Able to lift up to 25 lbs and respond to facility-related needs
