Spring Mountain Treatment Center is currently seeking a dynamic professional to join our team as a PRN Discharge Planner.
The Discharge Planner will serve as a member of a multi-disciplinary team supporting the hospital’s treatment program, philosophy, and assure the deliverance of quality treatment to patients and their family members. Provides discharge planning, case management, and psycho-educational groups to age specific populations. Stays informed of current community resources for discharge planning and conducts discharge planning groups as indicated.
JOB DUTIES/RESPONSIBILITIES:
- Coordinates care of assigned patients throughout their stay to plan and evaluate individualized safe and timely patient discharge needs.
- Works collaboratively with the physicians, staff nurses, and other Clinical Services Team Members to influence timely and smooth discharge with appropriate care and positive outcomes; presents alternative options and services as needed.
- Develops and coordinates an individualized discharge plan for the patient by utilizing team and chart information to determine the patient’s aftercare needs.
- Demonstrates current knowledge about full range of discharge options/community resources and utilizes them in the patient's discharge plan.
- Updating each patient/family contact to address discharge planning progress, a social service progress note will be entered outlining services provided when needed.
- Provides psycho educational or discharge groups as assigned. Following each group, documentation is made in the patient’s chart or on a group note.
- May attend on/off campus meetings related to patient, social work, and community issues.
- Attends educational in-services to continually update knowledge base related to behavioral health and social work issues.
- Understands and complies with the intent of the Performance Improvement Purpose and Procedures.
- Performs other duties as assigned.

Spring Mountain Treatment Center is a 110-bed mental health facility that provides acute crisis stabilization services for adults, teens, and children in a secure inpatient setting. Our individualized treatment includes a structured, therapeutic environment, intensive physician involvement and oversight, and daily group and educational programs. We provide a caring and healthy environment that gives individuals a chance to make positive changes.
ABOUT UHS:
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
- Bachelor's Degree required, Master's preferred.
- One to three years of relevant work experience in the behavioral health field required.
- Ability to effectively work with a multi-disciplinary team.
- Knowledge of treamtment plans.
- Experience in patient/family conflict resolution.
- Knowledge of Microsoft systems.
- BLS Basic Life Support certification
- New hires are required to attend 1 full week of New Employee Orientation on day shift, normally 8:00 am – 4:30 pm Monday through Friday
- Must successfully pass background check (includes criminal history, Child Abuse and Neglect Registry, and ABI/FBI fingerprint check)
