About the Role:
The Licensed Clinical Social Worker (LCSW) in Home Health plays a critical role in delivering comprehensive psychosocial assessments and interventions to patients within their home environment. This position focuses on supporting patients and their families by addressing emotional, social, and environmental factors that impact health and recovery. The LCSW collaborates closely with interdisciplinary teams to develop and implement individualized care plans that promote patient well-being and independence. Through ongoing evaluation and counseling, the LCSW helps patients navigate complex healthcare systems and access community resources. Ultimately, this role aims to enhance the quality of life for patients receiving home health services by providing compassionate, evidence-based social work care tailored to their unique needs.
Minimum Qualifications:
- Current and valid Licensed Clinical Social Worker (LCSW) license in the state of Connecticut.
- Master’s degree in Social Work (MSW) from an accredited program.
- Minimum of two years of clinical social work experience, preferably in home health, hospice, or related healthcare settings.
- Strong knowledge of psychosocial assessment techniques and care planning.
- Ability to travel to patient homes within Clinton Township and surrounding areas as required.
Preferred Qualifications:
- Experience working in home health or hospice care environments.
- Familiarity with Medicare and Medicaid regulations related to home health services.
- Training or certification in trauma-informed care or crisis intervention.
- Proficiency with electronic health record (EHR) systems and documentation standards.
- Bilingual skills or experience working with diverse patient populations.
Responsibilities:
- Conduct thorough psychosocial assessments of patients in their home settings to identify needs, strengths, and barriers to care.
- Develop, implement, and monitor individualized care plans in collaboration with healthcare team members, ensuring alignment with patient goals and clinical guidelines.
- Provide counseling and crisis intervention to patients and families to address emotional, behavioral, and social challenges related to illness or disability.
- Coordinate referrals to community resources, support services, and other healthcare providers to facilitate comprehensive patient care.
- Document all patient interactions, assessments, and care plans accurately and timely in compliance with regulatory and organizational standards.
- Participate in interdisciplinary team meetings to communicate patient progress, advocate for patient needs, and contribute to care planning.
- Maintain up-to-date knowledge of home health regulations, social work best practices, and relevant healthcare policies.
