Social Worker (Hospice) – Ponca City, OK

Hospice of North Central Oklahoma (HNCO) seeks a qualified Social Worker. Established in 1982, HNCO is a free-standing, community-owned, non-profit agency in Ponca City, Oklahoma. Nationally accredited through the Joint Commission, HNCO's mission is to provide quality end-of-life care at no expense to the patient while supporting their family and others in the community experiencing grief. Job purpose

Social Worker, under the direction of a physician, will provide social services based on the patient’s psychosocial assessment and the patient’s and family’s needs and acceptance of these services.

Duties and responsibilities

· Meet with patients and their family members in the patient’s home to provide emotional support for patients and patient family members as needed, including evenings and weekends.

· Available weekends and evenings to provide emotional support for patients and patient family members as needed.

· Provides resource/referral services to patients/families experiencing emotional, social and economic conflict and issues.

· Assess patient/family social and emotional factors and potential to cope with terminal illness and death within the first 5 days of service developing a plan of care to address the issues/problems identified.

· Provide information for the IDG through patient assessments in a timely manner as described by the policies and procedures.

· Assess patient/family ability to meet financial obligations and offer appropriate resources as indicated. Evaluates patient’s insurance coverage and options.

· Document all contact with patients/families in a timely manner.

· Assists in discharge planning from hospice services when patient no longer meets eligibility guidelines; a safety issue cannot be resolved; or the patient revokes hospice benefit.

· Participates in the Quality Assessment & Performance Improvement (QAPI) program as directed by the Director of Family Services.

· Participates in agency and community programs as requested to promote professional growth and understanding of hospice care.

· Maintains the standards of social service excellence in accordance with the principles set forth by the Oklahoma State Department of Health, NASW, NHPCO and agency policies and procedures.

· Exhibits hospice philosophy in all job related roles and activities.

· Assists with the bereavement program as needed and assigned by the Director of Family Services. (Including but not limited to, attending patient death during weeknights or weekends).

· Perform specific assignments as directed by the Director of Family Services.

Qualifications

· Bachelor’s Degree or Master’s Degree in social work from an institution accredited by the Council on Social Work Education.

· Or a baccalaureate degree in psychology, sociology, or other field related to social work and has 1 year of social work experience in a healthcare setting;

· Clinical experience in the areas of life-threatening and chronic illness, grief and loss, individual, family and group dynamics preferred.

· Written and oral communication skills.

· Experience and/or willingness to work with computer systems.

· Possess a current Oklahoma driver’s license and proof of vehicle insurance.
