Position Summary
We are seeking a compassionate and motivated Social Worker to join our Behavioral Medicine team full-time. This role is essential in helping patients navigate recovery, coordinate resources, and successfully transition to community living.

What You’ll Do

  • Complete psychosocial and mental health assessments

  • Develop and implement treatment and discharge plans

  • Provide individual counseling, crisis intervention, and patient advocacy

  • Connect patients with housing, healthcare, and recovery resources

  • Participate in interdisciplinary treatment team meetings

  • Document all clinical interactions in the Electronic Health Record (EHR)

  • Conduct suicide risk assessments and respond to crisis situations

What You’ll Need

  • Education & Licensure:

    • Master’s degree in social work (MSW) required

    • LSW or LCSW preferred

    • CPR certification preferred

  • Experience:

    • At least 1 year in behavioral health, substance use, or mental health settings

    • Inpatient psychiatric or addiction recovery experience a plus

  • Skills:

    • Knowledge of trauma-informed care, CBT, Motivational Interviewing, and harm reduction

    • Strong interpersonal and clinical documentation skills

    • Proficiency in EHR systems and Microsoft Office

    • Ability to thrive in a collaborative, multidisciplinary team
