Signature Healthcare at Home is seeking a Home Health Community Liaison to join our Puget Sound team. This is a field-based sales and relationship-building role (80–90% in the community) designed for someone who thrives on connecting with referral partners, meeting goals, and contributing to quality patient care.

What You'll Do:

• Serve as the primary liaison between our agency and community referral sources (hospitals, SNFs, ALFs, clinics, physician offices, etc.).
• Build and maintain strong relationships with key healthcare providers to drive referrals and admissions.
• Respond promptly to referral requests and inquiries, ensuring excellent customer service.
• Represent Signature at transitional care meetings, health system events, and community outreach efforts.
• Meet or exceed monthly referral and admission goals through proactive outreach and relationship management.

What We’re Looking For:

• 3–5 years in home health, hospice, or healthcare sales/marketing preferred.
• Existing network of referral contacts in the region is a plus

Skills/Education:

• Outstanding communication and presentation abilities (written & verbal).
• Tenacity, empathy, and a solution-oriented mindset.
• Associate or Bachelor’s in Marketing, Business, or Healthcare (preferred but not required).
• Valid driver’s license, reliable transportation, and ability to travel daily.
• Bonus: Clinicians (RNs, PTs, OTs, Social Workers) interested in transitioning to a liaison role are encouraged to apply.
