Overview
We are seeking a dedicated and compassionate Resident Care Coordinator to join our team. The ideal candidate will play a crucial role in providing high-quality care and support to individuals in various settings, including assisted living, nursing homes, and home care environments. This position requires a strong understanding of patient care, excellent communication skills, and the ability to work effectively with diverse populations, including seniors and individuals with developmental disabilities.
Responsibilities
- Develop and implement individualized care plans tailored to each resident's needs.
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, and meal preparation.
- Provide direct support in memory care and Alzheimer’s care settings.
- Monitor vital signs and document changes in patients' conditions using EMR systems.
- Collaborate with healthcare professionals to ensure comprehensive patient care.
- Conduct assessments and manage cases for individuals with disabilities, including autism.
- Facilitate conflict management and behavior management strategies as needed.
- Administer medications according to established protocols while adhering to HIPAA regulations.
- Maintain a clean and safe environment for residents, ensuring compliance with health standards.
- Drive clients to appointments or activities as required.
- Facilitate staffing schedule and training
Requirements
- Previous experience in assisted living, nursing homes, or senior care environments is preferred.
- Knowledge of medical terminology, first aid, and caregiving best practices is essential.
- Familiarity with dementia care techniques and memory care practices is highly desirable.
- Strong organizational skills and the ability to manage multiple tasks effectively.
- Excellent interpersonal skills for building rapport with clients, families, and team members.
