The Discharge Planner will coordinate the discharge process for patients in a correctional healthcare setting, ensuring smooth transitions of care by developing and implementing discharge plans. This role will collaborate with healthcare teams to assess patient needs, provide patient education, and arrange necessary follow-up care and services. The Discharge Planner will also ensure compliance with facility policies, healthcare regulations, and documentation standards.

How YOU can Make an Impact :

  • Monitor intake logs to identify patients with discharge planning needs and ensure all referrals for discharge planning support are followed up.
  • Collaborate with healthcare providers to identify patients requiring pre-release health planning and coordinate with behavioral health providers (including psychiatric and licensed mental health professionals) for those with complex needs.
  • Participate in multidisciplinary planning for specialized transitional programs, focusing on patients with chronic or serious health conditions (e.g., community-based mental health teams, HIV Jail Linkage programs).
  • Assist patients with application processes, confirming eligibility, and referring them to appropriate community resources and correctional facility program staff.
  • Coordinate appointments with community health providers as permitted by release schedules and ensure timely communication with those providers to facilitate smooth transitions.
  • Appropriately document all discharge planning activities in the patient health record, ensuring compliance with policies and procedures.
  • Collaborate with healthcare and jail staff to ensure effective patient orientation and pre-release planning.
  • Provide patients with educational materials regarding continuing care resources and community referral options.
  • Complete discharge summaries and coordinate with medical staff to ensure patients receive the required release medications from available inventory.
  • Develop and maintain procedures for handling designated patient classes (e.g., Veterans, treatment court clients), ensuring expedited verification of medications and coordination with court staff or providers as necessary.
  • Act as the liaison between MEDIKO, the Sheriff's Office, and community agency providers, such as the Veterans Administration, substance abuse programs, and mental health agencies.
  • Foster relationships with key providers to promote patient compliance, satisfaction, and positive health outcomes.
  • Attend select community meetings as needed to ensure coordination between the correctional facility’s medical program and community-based organizations.
  • Ensure proper documentation of discharge planning activities in inmate health records according to MEDIKO policies and procedures.
  • Prepare and maintain reports and logs summarizing pre-release planning activities, ensuring electronic records are complete and accessible for review.
  • Respond to requests from security and MEDIKO staff in accordance with facility and organizational guidelines, adhering to conduct and behavior standards set forth by the facility command staff.
  • Perform other related duties as assigned.


What we Ask of YOU:

  • Bachelor’s degree in a relevant field (e.g., Human Resources, Finance, Sales, Business Administration, Healthcare Administration, or related discipline) from an accredited institution preferred.
  • Additional certifications or specialized training may be required depending on the function (e.g., Certified Human Resources Professional (PHR) for HR roles, Certified Public Accountant (CPA) for Finance roles, etc.).
  • Minimum of 2-3 years of relevant experience in the specific functional area (e.g., HR, Finance, Sales, Marketing, Operations, etc.).
  • Experience in healthcare, correctional healthcare, or a regulated industry is preferred.
  • Demonstrated success in applying function-specific skills (e.g., employee relations for HR, financial reporting for Finance, or sales strategy for Sales) is required.
  • Knowledge of relevant industry-specific regulations, compliance standards, and best practices may be required, particularly for roles in highly regulated functions like HR and Finance.
  • Relevant industry certifications based on the role (e.g., PHR, SHRM-CP, CPA, etc.).
  • Active CPR/BLS certificate required (American Heart Association/American Red Cross guideline approved), if role is located in a correctional facility.
