This position is located onsite at one of our Tampa housing programs.
The Care Coordinator is a professional case management position focused on the coordination and linkage of services for individuals identified for needed behavioral health services- including substance abuse, mental health and/or housing.

Physical Requirements: Safety/Health Considerations:
Travel: Ability to drive to various locations daily using a company owned vehicle. This position does not require overnight travel.

Essential Functions
- Coordinates admissions between ACTS addiction receiving, outpatient or residential services and other community-based programs.
- Engages and motivates patients to enter or re-enter treatment settings to accomplish treatment plan goals.
- Ensure smooth transitions to the next level of care (warm hand-offs), is completed, to avoid gaps in service, and respond rapidly for needed support and/or intervention.
- Facilitate appropriate connection and linkage with peer support specialists, education, employment, housing and/or other needed wraparound services.
- May transports clients and/or materials to medical, legal, recreational and other community appointments that support their recovery.
- Attend multidisciplinary team meetings to review case status
- Attend court hearings, as needed
- Prepare documents including, but not limited to, service delivery reports.
- Prepares data reports related to placement success outcomes.
- Performs other related duties as assigned
- Bachelor's Degree in a human services related field
- Must possess valid Florida Driver License
- Must be able to fully participate in Welle (crisis intervention) and CPR/First Aid training.
- Must pass a Level II DCF, AHCA and/or DJJ background check.
- At least one (1) year of case management in a behavioral health setting
- Excellent verbal and written communication skills.
- Excellent interpersonal and customer service skills.
- Excellent organizational skills and attention to detail.

- Physically able to drive a vehicle for prolonged periods of time
- Ability to perform considerable reaching, stooping, and bending.
- Ability to lift, push, pull and carry
- Full participation in transportation-related training activities, including on-the-road driving
- Full participation in Welle (crisis intervention) and CPR/First Aid training.
