Job Summary
Performs various supervisory activities for scheduling appropriate caregivers, timecard processing, and other office coordination duties. Will assist in the ongoing training of caregivers as well as the overall satisfaction of clients. Ensures that all referrals are received in an accurate, detailed manner and are properly handled. Will be responsible for performing general office duties. Care coordinator is responsible for assisting with the on-call duties for the Administrative team.
Essential Functions
· Responsible for ensuring that all shifts are staffed in a timely manner
· Proactively staff client cases with caregivers who will fit the client’s plan of care outline in order to reduce the number of faces introduced to each client and limit service interruptions
· Serve as a liaison between office and clients
· Schedules and coordinates day to day activities of caregivers and ensures caregivers have pertinent information in order to provide best level care to clients
· Receives referrals / inquiries and handle in manner which will promote the Right at Home brand to increase service hours, schedule in home assessments for Community Managers
· Assist with the ongoing training and development of field caregivers by performing team lead visits and mentoring shifts as needed; this is inclusive but not limited to coaching, promotion of effective behaviors, and redirection of ineffective behaviors
· Properly document any necessary disciplinary situations with caregivers in order to provide audit trail for the personnel file
· Maintains documentation of caregiver performance in Wellsky in order to ensure complete records for homecare associates
· Manage core client groups in order to increase overall client satisfaction; this is inclusive but not limited to site visits, phone calls, and other types of specialized attention in order to increase client retention rates
· Provide superior service using professional mannerisms via various forms of communication; respond promptly and courteously
· Perform Administrative on-call coordinator duties as needed (1x every 5 weeks)
· Assist with sales, marketing, and public relations efforts as needed
Additional Functions
· General office and clerical functions, including filing
· Ad hoc duties as assigned by the Administrator and Owner
Education, Experience, Knowledge, Skills, Abilities and Availability
· High school graduate or equivalent education
· Basic office and computer skills, organizational abilities, and medical terminology
· Display high level of interpersonal relations abilities
· Professional written and oral communication
· Ability to work independently
· Demonstrate sound judgement and reasoning
· Work well with others by conducting self in a professional manner
Working Conditions/Environment
· Works primarily out of the local office
· 4 days per week; 40 hours
o Weekly rotation of days (9:00am to 7:00pm)
o Saturday from 8am to 6pm - once every 6 weeks
