Care Manager Job Description

JOB SUMMARY:

Provide assistance for the AFC member in accordance with the AFC care plan and physician orders.

Employee performance is evaluated by supervisors on an on-going basis. Areas to be considered include: Job knowledge, Performance, Attitude, Initiative, Cooperation, Use of Equipment/Supplies,

Safety, and Sanitation. Employees receive appropriate suggestions for improvement when necessary and are recognized for good performance.

JOB QUALIFICATIONS:

Education: A bachelor’s degree or a Social Worker license from the Massachusetts Board of Registration in Social Work and at least two years’ recent experience working with elders or persons with disabilities.

Licensure: Driver’s License.

Skills: The Case Manager must have excellent organizational and time management skills, problem solving, ability to multi-task, be able to communicate effectively with staff, nursing personnel, PCP, members, families, administration, and outside organizations.

Medical: MD Clearance letter, TB test.

Transportation: Reliable transportation. Valid and current auto liability insurance.

PHYSICAL REQUIREMENTS:

1. Read 10 point or larger type,

2. Speak clearly in a manner understood by most people,

3. Sit, stand, bend, lift, kneel and move intermittently during working hours,

4. Climb stairs,

5. Travel to prospective member’s residence,

6. Lift and transfer members,

7. Work in the members’ home environment as well as the office,

8. Relate to and work with ill, disabled, emotionally upset and, at times hostile people.

DUTIES AND RESPONSIBILIES OF THE CARE MANAGER:

1. Conduct initial social history and ongoing psychosocial assessments, at least annually, of the member’s appropriateness for AFC.

2. Evaluate support, and train AFC direct care aides.

3. Assist with the development and implementation of the member’s interim and final AFC plan of care with input from the member or responsible party, all members of the MDT, and other individuals designated by the member.

4. Participate in the development, implementation, and ongoing review of the AFC plan of care.

5. Visit the member on the day of the admission, after one week, three weeks, 30 days, then every other month.

6. Assist member with obtaining information, coordination and access to other health care, community, and supportive services.

7. Provide timely assistance & response to urgent or emergency needs of members.

8. Assist with making referrals to appropriate service providers if the member requires services other than those provided by the AFC provider.

9. Complete a care management progress note for each on-site visit or upon a significant change and submit all required paperwork in a timely manner according to agency policy and procedure.

10. Report changes in the member’s condition to the member’s AFC nurse.

11. Handle case assignments, review case progress and determine case closure.

12. Review/Update every 6 months or as changes occur the Individual Service Plan for the member.

13. Report changes in member’s medical, social, financial, or family situation to the AFC Nurse and/or Program Director.

14. Develop in conjunction with the MDT, the AFC direct care aide, and the member or responsible party, an emergency backup and personal care contingency plan for each member receiving AFC that includes an alternative plan for the member if the AFC provider is temporarily unable to provide care.

15. Participate in discharge planning and implementation.

16. Conduct an initial review of the suitability of the member’s home and review at least annually the suitability of the qualified setting, report, and document if any changes.

17. Provide case management.

ADDITIONAL DUTIES:

18. Attend and participate in continuing educational programs designed to keep current of changes in the profession and home care regulations and standards.

Job Types: Part-time, Contract

Pay: $50.00 per hour

Expected hours: 10 – 30 per week
