About Job

This role involves conducting psychosocial assessments to evaluate patient needs and supporting patients and families through hospitalization or new diagnoses.

  • Provides crisis intervention, grief counseling, facilitates family meetings, and documents patient information.
  • Collaborates with care teams, including specialists, to develop treatment plans.
  • Creates and maintains a database of community-based psychosocial resources for patients and staff.
  • Requires a Master’s in social work (MSW), knowledge of mandatory reporting laws, and preferably two years of experience in social services or discharge planning, with strong preference for palliative care experience.
  • Must have excellent communication skills, proficiency in Microsoft Office and electronic medical records, and knowledge of medical terminology.
  • Serves all age groups, with a salary range of $54.18 - $72.04/hour.
  • Physical demands are mostly sedentary, with occasional lifting and walking; accommodations will be provided for qualified individuals with disabilities.
