Responsibilities:

· Recruit, orient and train urgent care team members;

· Provide and document clinical and administrative supervision;

· Lead and document team meetings, case consultations and debriefings with urgent care team members;

· Provide regular and consistent feedback and coaching as needed;

· Conduct competency reviews and performance evaluations;

· Complete monthly clinical record reviews;

· Practice, model and demonstrate effective strategies of client engagement and reengagement;

· Work with team members to facilitate a thorough and effective screening and assessment process, transitional care and discharge planning;

· Work closely with the other TGC’s programs to facilitate client engagement in clinically appropriate aftercare services following urgent care services and intervention;
