Principal Responsibilities:

The YM & YWHA of Washington Heights and Inwood is seeking a social worker for the Center for Aging in Place, who is passionate about working with older adults. This position requires strong clinical assessment skills and a knowledge of aging-related issues.

Home Visits, Washington Heights, Inwood & Riverdale

RESPONSIBILITIES:

Conducting outreach and comprehensive psychosocial assessments of older adults.
Referring older adults to the appropriate Y Aging in Place services and other programs.
Conducting home visits for initial assessments, care planning and ongoing support.
Providing supportive counseling, advocacy and resource coordination.
Maintaining electronic records using an online relational database.
Engaging in regular supervisory and team meetings.

POSITION TYPE / EXPECTED HOURS OF WORK:

Part-time, FSLA Non-Exempt, onsite position
This is a part time position, 21 hours per week

COMPENSATION:

$30 per hour worked

WORK ENVIRONMENT:

Work is primarily performed in a community center environment

Minimum Qualifications:

REQUIREMENTS:

LMSW required; LCSW preferred
Minimum of two years of experience working with the geriatric population
Proficient in Microsoft Office suite and other technology
Excellent communication and interpersonal skills with a focus on the principles of trauma informed care
Ability to work independently and as part of a team

WORK AUTHORIZATION & CLEARANCE

Documentation of U.S. citizenship or eligibility to work in the U.S.
Must pass a background check and other pre-employment requirements

Salary:

$25,000 - $35,000
