Job Summary

The Housing Care Coordinator works in coordination with CareOregon’s Regional Care Teams, community partners and other agencies to provide housing resources and eviction prevention and basic health plan care coordination for high risk CareOregon members. This position provides intake functions, general physical and behavioral health plan navigation, and care coordination including assessing member needs, developing individualized care plans, coordinating with community providers and with members. The primary focus of this position is to doing outreach in the community to high-risk members with unstable housing and acting as a consultant to field staff with the goal of assisting members to find and maintain stable housing. This position requires critical thinking and independent judgment as well comfort working with the organization’s most vulnerable members.

Essential Responsibilities

Housing Navigation

  • Provide housing navigation for eligible members and families.
  • Assess members for housing needs, identifying barriers and strengths to inform a housing care plan.
  • Pursue affordable housing options for clients throughout the tri-county area.
  • Assist clients with accessing affordable housing throughout the tri-county area.
  • Assist clients in completing all appropriate forms for obtaining housing.
  • Attend all meetings required to best work with landlords, resident services and building staff.
  • Assist clients with developing working relationships with landlords, resident services and building staff.
  • Attend required meetings and trainings.
  • Advocate for clients when necessary; attending meetings such as landlord tenant or court with client.
  • Help clients move or make improvements to their dwelling to insure housing stability.
  • Assist members in establishing/engaging with community providers.
  • Maintain a caseload of members with ongoing housing needs.

Care Coordination

  • Provide general health care coordination to eligible members and families.
  • Facilitate communication between members, their support systems, other community-based partners, clinical care providers and other CareOregon departments, and ensure housing care plans are shared, as appropriate.
  • Provide support as appropriate to clinical Care Coordination staff involved with the member.
  • Refer members to appropriate agencies for community supports.
  • Take independent actions to address members’ identified needs including, but not limited to: scheduling provider appointments, arranging for transportation, arranging for an interpreter when needed, assisting with Health Related Services Flex requests, verifying needed services are provided, referring to community resources and locating members.
  • Develop working partnerships with community partners regarding member needs and care plans.

Assessment and Care Planning

  • Assess for and identify housing and care coordination needs.
  • Identify risk factors and service needs that may impact member outcomes and address appropriately.
  • Utilize a trauma-informed approach to provide member-centric support.
  • Use motivational interviewing to coach members toward improved health care behaviors and self-management in support of their housing goals.
  • Utilize housing assessment information to develop individualized care plans for assigned members.
  • Implement care coordination housing plan in collaboration with member, providers, case workers and other relevant parties.

Compliance and Administration

  • Maintain compliance with Coordinated Care Organization requirements.
  • Complete initial and subsequent documentation and processes in care coordination activities and when supporting clinical care coordinators in creation of plans of care, according to the care coordination timeline of activities.
  • Maintain working knowledge of COA and OHP benefits.
  • Report member complaints to Appeals and Grievance team for investigation and follow-up, per protocol.
  • Participate in quality and organizational process improvement activities and teams when requested.
  • Participate in work-related continuing education when offered or directed.

Required

Experience and/or Education

  • Two years’ relevant case management experience in a housing related field OR any work experience and/or training that would likely provide the ability to perform the essential functions of the position.
  • Valid driver’s license, a personal vehicle and safe driving record is required

Preferred

  • Bachelor’s degree with major course work in social science, or a related field

Knowledge, Skills and Abilities Required

Knowledge

  • Knowledge of community resources
  • Knowledge of barriers to care such as language, cultural factors, transportation, ability to self-manage and psychosocial issues and bring those to the attention of the care team
  • Knowledge of Oregon Health Plan (OHP) benefit package
  • Knowledge of landlord tenant law, Shelter Plus Care voucher system, Section 8, and any other programs as deemed necessary

  • Ability to see, read, and perform repetitive finger and wrist movement for at least 6 hours/day
  • Ability to hear and speak clearly for at least 3-6 hours/day
