We value our patients, our clients, and our staff and are currently seeking a full-time Social Worker/Case Manager to add to our team in Placer County. Our 16 bed high acuity psychiatric unit offers an opportunity for challenge, growth and career satisfaction.

Job duties include, but are not limited to: case management of mentally ill adult patients, providing counseling as needed, assisting the clinical team in developing and modifying treatment plans, coordinating treatment and discharge plans, acting as a liaison in court proceedings, leading therapeutic groups, and coordinating with the clinical team, county caseworkers, and other agencies as needed.

This is a full-time position. 40 hours per week available. Schedule may include Weekends.

Qualifications:

• BSW or MSW required
• LCSW or LMFT preferred
• Experience working in a psychiatric inpatient setting or with mental health clients in an outpatient setting is preferred.
• Strong oral and written skills.
• Team player.

• Dynamic self-starter
• Must pass mandatory Department of Health Care Services background screening and a federally-compliant drug screen.
• Covid-19 vaccination or approved exemption required

Job Duties include:

• Representing the hospital in all Certification Review, Riese, and Writ hearings
• Serving as the point of contact for Superior Court
• Engaging with patients
• Serving legal holds
• Conducting psychosocial assessments in compliance with laws and regulations.
• Participating in treatment team and collaborating with physicians, nurses, and community partners in care and discharge planning.
