About the Role: The Greater Los Angeles Health Care System is currently recruiting two Senior Social Workers to join our Patient-Aligned Care Team (PACT) at the Antelope Valley VA Clinic, located in Lancaster CA. As a key member of a comprehensive team dedicated to providing primary care to Veterans, you will play a crucial role in prevention, health promotion, coordination, and chronic disease management. In this role you will provide services to Veterans in the PACT, addressing complex psychosocial problems of Veterans, families, and significant others. Proactively reach out to high-risk Veterans to offer case management and prevent psychosocial crises.

Key Responsibilities:

  • Educate and train Veterans on the referral processes and services provided, focusing on case management and reducing acuity levels using evidence-based practices.
  • Assess psychosocial problems causing distress, such as adjustment to medical conditions, untreated mental health or substance abuse issues, economic instability, legal problems, and inadequate social support, housing, and transportation.
  • Assist Veterans and their families in understanding contributing factors to their problems, discuss solutions, and encourage positive, lasting changes.
  • Make referrals to community agencies and coordinate with VA programs/staff for comprehensive problem-solving.
  • Engage case management with outpatient interdisciplinary team to meet treatment goals.
  • Conduct comprehensive assessments and develop treatment plans sensitive to Veterans' diverse needs.
  • Perform full range of clinical services, including individual, group, and family interventions.
  • Provide on and off-site crisis intervention and outreach to maximize the stabilization and integration of Veterans in the community.
  • Develop and implement methods for measuring the effectiveness of social work practice and services, utilizing outcome evaluations to improve treatment services.

Work Schedule: Monday – Friday, 8:00 am-4:30pm

Qualifications:

  • Must be a U.S. citizen
  • Master’s degree in social work from a school of social work fully CSWE accredited
  • Must possess a current, full, unrestricted license or certified by a state to independently practice social work at the master’s degree level.
  • Must have two years of experience post advanced practice clinical licensure.
