Responsibilities
Under the leadership of the Social Work Director, the licensed Social Worker provides professional clinical social work intervention to patients diagnosed with cancer and other blood disorders. The position provides counseling, crisis intervention, patient advocacy, practical care and referrals to appropriate community resources for patients and their families/caregivers. This position works with a multi-disciplinary team and serves as a liaison for patients/families with medical personnel and community resources.

Responsibilities
Under the leadership of the Social Work Director, the licensed Social Worker provides professional clinical social work intervention to patients diagnosed with cancer and other blood disorders. The position provides counseling, crisis intervention, patient advocacy, practical care and referrals to appropriate community resources for patients and their families/caregivers. This position works with a multi-disciplinary team and serves as a liaison for patients/families with medical personnel and community resources.
- Performs and documents psychosocial distress assessment on new cancer patients focusing on current stressors.
- Provides crisis intervention counseling, education and community resource linkages
- Provides practical care including home services, assisting with transportation and working closely with the Resource Navigator.
- Participates in the psychological, social, emotional and financial care of patient in coordination with the care team.
- Must possess a degree in: Social Work
- Licensed in the State of Maryland as a an LMSW, LCSW-C preferred
- Experience in providing clinical social work services in a medical of mental health setting
- Accurate verbal and written expression in the English language
- Work organization, problem solving
- Psycho-social counseling

- Must be able to perform:
- Bio-psychosocial assessments
- Crisis counseling
- Community resource referrals
- Documentation
- MD drivers license and use of car during the working hours may be required for agency visits and professional meetings.
