Responsibilities
As a member of the Social Work Department, the social worker provides direct social work services to patients and families identified as having psychosocial or discharge planning needs. Partners with the patient, family, healthcare team, and community agencies in the safe transition of care to the next most appropriate level. Must be able to communicate effectively with physicians, staff, patients and families and community organizations. The social worker must be able to handle multiple priorities and have good organizational skills.

Responsibilities
As a member of the Social Work Department, the social worker provides direct social work services to patients and families identified as having psychosocial or discharge planning needs. Partners with the patient, family, healthcare team, and community agencies in the safe transition of care to the next most appropriate level. Must be able to communicate effectively with physicians, staff, patients and families and community organizations. The social worker must be able to handle multiple priorities and have good organizational skills.
Duties Include
- Performs psychosocial assessments and provides accurate community resource linkage as part of discharge plan, identifying correct resources, educating patient on use of resources and completing all requirements of successful referral.
- Employs a range of clinical interventions such as crisis intervention, care coordination, information gathering and referrals. Advocates on behalf of patients and families to gain access to services and resources.
- Develops and implements effective discharge plans for all patients on referral, involving patient and family in formation of plan and achieving the discharge within the appropriate length of stay.
- Participates effectively in interdisciplinary plan of care, including communication with physicians and staff, and participation in multidisciplinary rounds.
- Documents social work interventions accurately, consistent with departmental guidelines for content and timelines.
- Must possess a degree in: Social Work
- MSW with 2 years experience in healthcare setting preferred
- Licensed in the State of Maryland as a an LCSW, LMSW, or LCSW-C
- Accurate verbal and written expression in the English language
- Work organization, problem solving
- Psycho-social counseling
- Conflict resolution
- Identification of crisis situations er
- Must be able to perform:
- Bio-psychosocial assessments
- Crisis counseling
- Community resource referrals
- Documentation
- MD drivers license and use of car during the working hours may be required for agency visits and professional meetings.
