Title: Care Coordinator
Overview: The Care Coordinator is the link to ensure a continuity in coordination of
client care through their collaborative interaction with all the providers and community
relationships involved in that care – including but not limited to – physical health,
behavioral health and social service providers.
• Gathers information for treatment plans, including social and cultural factors that
influences all aspects of health.
• Conducts visits with client and family in school, home and community-based
settings.
• Coordinate services utilizing the wraparound delivery model.
• Collaborate with other members of the multidisciplinary team, as appropriate, to
develop treatment goals for the client.
• Participate in multidisciplinary client care conferences discussing information
gathered from client assessments and treatment plans.
• Complete all clinical documentation within 24 hours of services provided.
• Co-facilitate groups in schools.
• Participate in clinical supervision and team meetings.
• Provide hybrid services as needed (virtual/in person).
Schedule: Up to 20 hours per week Monday – Friday.
Education: Bachelor’s degree in Social Work, Counseling, or Psychology preferred. A
candidate with 3-5 years experience working with child and families will be
considered.
Licensure: Not required.
Job Type: Part-time
Pay: $23.00 - $27.69 per hour
