Position Overview

As a member of the multi-disciplinary team, the Social Worker - Personal Care Home is responsible for providing social work services to residents and their families, including counselling, consultation, education, and advocacy. The incumbent identifies concerns within the facility and collaborates with staff to implement appropriate interventions to meet residents' psycho-social needs.

The incumbent exercises initiative and independent judgment to determine work priorities, methods, and actions on unusual matters. The position aligns with the mission, vision, values, and policies of Southern Health-Santé Sud.



Experience

  • One (1) year of previous experience as a Social Worker in a long-term care setting.
  • Experience working within a multi-disciplinary team.


Education (Degree/Diploma/Certificate)

  • Graduate of an accredited Faculty of Social Work.


Certification/Licensure/Registration

  • Current registration with the Manitoba College of Social Workers.


Qualifications and Skills

  • Knowledge of Personal Care Home Standards regulation.
  • Ability to respect and promote a culturally diverse population.
  • Proficiency in both official languages is essential for targeted bilingual positions.
  • Ability to maintain confidentiality in all formats.
  • Good work and attendance record.
  • Valid Class 5 driver’s license, insured vehicle, and liability insurance of at least $1,000,000.00.


Physical Requirements

  • Ability to meet the physical and mental demands of the job.
  • No hazardous or significantly unpleasant conditions.
  • Occasional evening and weekend work may be required.
  • Travel to other regional facilities may be necessary.

This term position may end earlier as outlined in your collective agreement.
