Job Summary
We are seeking a dedicated and compassionate Licensed mental health counselor (LMHC) or Licensed Clinical Social Worker (LCSW) to join our team. The ideal candidate will provide high-quality clinical counseling and support to individuals and families, focusing on enhancing their overall well-being. This role involves working closely with patients to assess their needs, develop treatment plans, and deliver effective interventions in a supportive environment.

Duties

• Conduct comprehensive assessments of patients' mental health and social needs.
• Provide clinical counseling and therapeutic services to individuals, families, and groups.
• Develop and implement personalized treatment plans based on patient assessments.
• Manage case documentation, ensuring accurate medical records and adherence to medical terminology standards.
• Collaborate with healthcare professionals to coordinate patient care and ensure holistic support.
• Utilize applied behavior analysis techniques as appropriate for patient interventions.
• Maintain up-to-date knowledge of behavioral health trends and best practices in social work.
• Advocate for patients' needs within the healthcare system and community resources.
• Participate in ongoing professional development to enhance clinical skills and knowledge.

Qualifications

• Master’s degree in Social Work from an accredited institution.
• Valid state licensure as a Clinical Social Worker (LCSW).
• Experience in home community care, clinical counseling, case management, or related fields is preferred.
• Strong understanding of medical documentation practices and medical terminology.
• Proficient in behavioral health practices with a focus on patient care.
• Excellent communication skills, both verbal and written.
• Ability to work collaboratively within a multidisciplinary team environment.
