About ThedaCare!
Summary :
The Advance Care Planning (ACP) social worker provides support to the ACP Specialist to development, implement, optimize and integrate ACP across clinic, hospital, nursing home, home and community setting across service lines. Works directly with team members, patients and community members to ensure everyone over the age of 18 has ACP knowledge and a Power of Attorney for Health Care document. Provides education and support in the hospital and ambulatory setting while documenting in EPIC as necessary.

Schedule:
Full time, benefited position
40 hrs/week
Travel to various sites around the system doing ACP (advance care planning) clinics.
Help complete advance directives in the inpatient setting
Key Accountabilities
Facilitate monthly group classes and weekly individual ACP clinics.
Coordinate ACP volunteers in the hospital and ambulatory settings.
Provide outreach and information to community members/patients in regards to ACP.
Address/correct incorrect POA-HC documents.
Facilitate team member education at various ThedaCare locations.
Facilitate patient/community informational sessions at various ThedaCare locations.
Schedule patients directly with ACP resources within the system.
Support ACP Facilitators throughout system through education resources.
Qualifications
Bachelors or Masters degree in social work from an accredited university.
Licensed/Certified as Clinical Social Worker (LCSW), Advanced Practice Social Worker (APSW), Certified Independent Social Worker (CISW), or Certified Social Worker (CSW)
Certified as a Respecting Choices ACP Facilitator (or willing to complete within the first six months of employment)
Experience in health care preferred.
Physical Demands
Ability to move freely (standing, stooping, walking, bending, pushing and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance
Category 2: Job classification is not exposed to blood borne pathogens (blood or bodily fluids) while performing job duties
Work Environment
Normally works in climate controlled office environment
Frequent sitting with movement throughout office space
