Ark Healthcare and Rehabilitation at Branford Hills, seeks a Full-TIme, PDPM Care Coordinator to join our team of Nursing Professionals. Our 190-bed skilled nursing facility, located in the Heart of Branford, CT is a leading provider in Short-Term Rehabiliation and Long Term Care.

Schedule:

Full time Monday-Friday Day Position

Job Requirements include but not limited to:

• Liaison with insurance case managers
• Oversees and coordinates sub-acute residents discharge planning.
• Collaborates with the interdisciplinary team to set sub-acute resident's goals of care and achieve desired outcomes

Qualifications:

• Nursing or Therapy Back round required.
• Experience in Long term care required.
• Strong organizational and communication skills
• Attention to detail and good follow-through skills.
• Ability to prioritize multiple tasks

The ideal candidate will have experience with working with insurance case managers and experience with case management, PDPM and possess skills and knowledge to maximize reimbursement.
