Salary: Starting at $21-$24 based on experience.


Statement of Purpose: While living well with an HIV diagnosis, able to help others navigate the challenges of living with HIV. Create rapport, share experiences, and assess clients' needs. Assist with client education to improve their adherence to medical protocol, their coping skills, and quality of life.


Primary Tasks:

  1. Coordinate the day-to-day activities of the Medical Peer Navigators.
  2. Assure appropriate distribution of workload for Medical Peer Navigators.
  3. Assure that new staff are appropriately trained and oriented to patient care programs.
  4. Report on any staff performance issues or patient complaints to CAN Leadership for appropriate actions.
  5. Educate community partners about CAN Community Health's services. Foster improved working relationships between CAN and community
  6. Assess and address clients' knowledge, attitudes, and beliefs regarding HIV
  7. Facilitate peer sessions that support strategies to overcoming barriers to care
  8. Provide HIV education and sexual risk reduction counseling regarding HIV and sexually transmitted infections
  9. Documentation of all client interactions and activities
  10. Plan and attend peer related groups and events
  11. Ability to work with a multidisciplinary team of medical professionals including case management and patient care coordinators
  12. Comfortable with disclosing HIV status to clients and relevant staff
  13. Educate clients about the 340B program and their pharmacy choices
  14. Educate clients about support groups, educational opportunities, etc.
  15. Assess clients' adherence, willingness, and abilities regarding antiretroviral therapy
  16. Assess clients' needs for services including dental, mental health, and case management
  17. Discuss relationship status and goals with clients
  18. Provide community outreach (as assigned) to support recruitment, retention and re-engagement of CAN patients
  19. Establish and maintain knowledge of community resources and referrals
  20. Attend community based committee meetings to gather information for referrals.
  21. Participate in local and state-level HIV planning efforts.

Secondary Tasks:

  1. Promotes and practices CAN's values and mission, and adheres to all policies and procedures
  2. Maintains confidentiality regarding patient and/or family in accordance with professional and departmental standards
  3. Participates in continuous quality improvement of services rendered to CAN clients
  4. Develops and fosters networking relationships with community and support resources
  5. Contact clients to remind them of their appointment(s)
  6. May accompany client to appointments outside of CAN
  7. Participation in the HIV Speakers Bureau is encouraged
  8. Stay up-to-date with the new data/science in the field of HIV
  9. Submit reports in a timely fashion

Physical Requirements:

  1. Requires repeated walking, bending, stooping, reaching, and moderate lifting of up to 50 pounds
  2. Requires visual and auditory acuity
  3. Requires ability to use computers and productivity software such as Microsoft Word and Microsoft Power Point.


Knowledge, Skills and Abilities Required:

  1. Adept at navigating cultural differences
  2. Adept at working with persons of color, gender and the lesbian/gay/bisexual/transgender/queer community
  3. Understanding of how HIV is transmitted and prevented as well as familiarity with antiretroviral therapy
  4. Knowledge of HIV medical terminology, procedures, medications and accepted treatment practices.
  5. Ability to work independently with accountability
  6. Exercise sound judgment, discretion, and professionalism
  7. Sound communication and interpersonal skills with patients, visitors, fellow staff members, and the supervisory team.
  8. Ability to utilize problem-solving techniques
  9. Bi-lingual is preferred
  10. Must be able to operate a motor vehicle and have it available with valid insurance and driver's license
  11. Familiar with Microsoft Office and use of social media


Requirements:

Education/Professional Experience:

  1. High School / GED
  2. HIV/AIDS 500/501 courses within one year of hire
  3. Other relevant HIV experience is required

Must be able to pass a Level I background check (a Level II background may also be required).
