About the Position:
The RN Care Coordinator position plays a critical role in managing patient care in our ambulatory clinics, with focused efforts on the management of diabetes and hypertension health conditions. This role involves patient education, care plan development, and patient advocacy by coordination of care and services. The role is primarily Mondays through Fridays, 8 am-5 pm, with no weekends or holiday requirements. $5,000 Hiring Bonus — paid in two increments

What You’ll Do
As an RN Care Coordinator, you’ll serve as a trusted advocate, educator, and collaborator — helping patients navigate their healthcare journey and achieve better health outcomes.
Care Coordination & Management
- Lead and collaborate with the patient-centered care team to ensure top-quality, preventive-focused care
- Supervise a care coordination team (Patient Navigator, Unit Clerk, Medline Assistant)
- Support high-risk patients through assessments, care planning, and, when needed, home or hospital visits
- Educate patients, families, and caregivers on the Patient-Centered Care Model
Service Coordination & Advocacy
- Provide case management, client advocacy, and goal-based care plans
- Build relationships with patients and families to encourage treatment adherence and self-management
- Serve as the go-to resource for patients regarding medical and dental benefits
Monitoring & Reporting
- Track and document patient progress, addressing barriers to care
- Maintain strict HIPAA compliance and patient confidentiality
- Collect and analyze data to improve services and prepare monthly reports
Other Responsibilities
- Take on additional duties that support patient care and clinic success
What We’re Looking For
- Experience: At least 2 years in community health, public health, outreach, social work, or related fields — with progressively responsible leadership experience.
- Specialty Knowledge: Pediatric experience required.
- Leadership: Background in care coordination or care management in a clinical setting.
- Technical Skills: Comfortable with data entry and basic computer use.
- People Skills: Exceptional communication and interpersonal abilities to connect with individuals from diverse backgrounds.
- Team Mindset: Ability to collaborate with clinic management, providers, and staff for seamless patient care.
- Licensing: Graduate of an accredited RN program, with a current license to practice in Pennsylvania.
Preferred Qualifications
- Familiarity with The Joint Commission regulations
- Bilingual abilities in Spanish, Nepali, or French
