In this role, you’ll be the heart of our resident care operations—partnering with residents, families, and staff to create personalized care plans, support our care team, and ensure our community runs smoothly and compassionately.

What You’ll Be Doing

  • Creating individualized, person-centered care plans that reflect each resident’s preferences and needs.

  • Coordinating with families and our care team to make sure everyone is informed and supported.

  • Helping to hire, train, and guide care staff to maintain the highest standards.

  • Overseeing medication management, ordering supplies, and keeping everything organized.

  • Ensuring compliance with state regulations and community policies.

  • Supporting quality assurance efforts and participating in incident follow-ups.

  • Pitching in with scheduling, budget oversight, and operational needs.

What We’re Looking For

  • Experience in long-term care (assisted living, memory care, or skilled nursing preferred).

  • Strong leadership, communication, and organizational skills.

  • Knowledge of Oregon state regulations for resident care and medication management.

  • A team player who can also thrive working independently.

  • Someone who values residents’ rights, safety, and dignity above all.
