Care Coordinator
The primary role of the Care Coordinator (CC) is to coordinate and monitor treatment engagement and utilization review.
- Making introductory calls to newly transferred clients to explain their role and review the treatment plan.
- Review initial signed consent forms to ensure they are completed and uploaded.
- Coordinate new treatment referrals, if needed, including obtaining new releases.
- Ensure all aspects of the treatment intake are in place, including transportation.
- Complete reminder and follow up calls to clients about their intake appointments.
- Complete all aspects of utilization review including regularly scheduled Continuing Service Reviews (CSR) to determine the next steps for the case including ongoing management at the same level of care, a different level of care or case closure.
- Present cases at the statewide DCP&P consortia, CC will all be trained to present at all statewide DCP&P consortia.
- May initiate referrals for Intensive Case Management (ICM) and collaborate with ICM staff.
- Send outreach letters to clients who have left treatment.
A Master’s Degree in counseling, social work, or related field with an LAC/LPC, LCADC, or LSW/LCSW is required. Must have a minimum of two years of co-occurring disorder experience and applied knowledge of The ASAM Criteria, 3rd Ed., and the DSM-5-TR.
Position can be based out of either Robbinsville or Sewell, NJ depending on candidate's residence and can be a hybrid or fully work from home role once trained.
