Overview:
The Acute Rehabilitation Program Coordinator is an integral part of a multidisciplinary collaborative effort to identify and deliver quality and cost efficient healthcare to patients on acute rehabilitation. The employee is a liaison between persons served, the rehabilitation team, and external resources. Serves as one of the primary facilitators for patient referrals from internal and external referral sources to ARU. In this role the rehabilitation referral coordinator will be expected to interact with discharge planners, insurance carriers, physicians, patients, family members and other related parties who may be involved with or inquiring about the rehabilitation programs.
Responsible for providing pre-admission intake assessments for acute inpatient rehabilitation services. Also have primary responsibilities in the day to day areas of patient evaluation using the IRF-PAI, ensuring timely and accurate completion of the IRF-PAI/CARE tool. Responsible for inputting the data, and electronically transmitting the data to Medicare/UDS. Assures the documentation meets criteria for Finley and CMS regulations.
Rehabilitation program coordinator will work closely with Rehab Nurse Manager to ensure leadership standards are met. This will include reporting and interpreting rehabilitation data, preparing required reports, and active involvement in all CARF requirements. In addition, this employee is expected to adhere to and exemplify the UnityPoint Focus Values
