Guide Program Care Coordinator
At the Mount Washington Valley Adult Day Center, we are passionate about serving the needs of community members with memory loss and providing their families and caregivers with respite and peace of mind. We have an opening for a special person to work in the Guide Dementia Program while working closely with the care management team. The MWVADC is one of only 3 healthcare organizations in New Hampshire to have been chosen by the Centers for Medicare & Medicaid Services to offer this exciting program. The Guide program is a comprehensive dementia care program funded by Medicare to improve the quality of life for people with dementia, reduce strain on their unpaid caregivers, and enable people with dementia to remain in their homes and communities. The Care Coordinator is the primary point of contact for those enrolled in the program. The responsibilities include: conduct initial home visit assessment and caregiver assessment, develop person-centered care plans, document all interventions in the electronic health record, maintain monthly contact with participants, help coordinate additional respite services and caregiver education.
The successful candidate will have proven organizational, presentation and interpersonal skills in a relevant previous position. Prior experience with program development and implementation or proven accomplishments in a related therapeutic position is essential.
