Are you a Licensed Practical Nurse looking to bring your clinical expertise into a modern, innovative telehealth setting? Telegra MD, a leading Telehealth company, is seeking an LPN with exceptional organizational skills and a passion for patient care to serve as both a clinical subject matter expert and a Patient Care Coordinator. In this dual role, you will provide consistent clinical guidance to support our telehealth operations, ensure exceptional patient experiences, and serve as the link between clinical and administrative functions.

Key LPN Responsibilities:

• Serve as a clinical resource and subject matter expert for the PCC team, offering guidance on patient care protocols, medical terminology, and best practices.
• Perform basic clinical assessments, document patient information accurately, and ensure compliance with telehealth and nursing regulations.
• Review and process patient records for accuracy and completeness, ensuring proper follow-up care is coordinated.
• Assist providers in preparing for telehealth appointments, including reviewing patient histories and confirming readiness for virtual visits.
• Maintain up-to-date knowledge of clinical guidelines, telehealth best practices, and state nursing regulations.
• Collaborate closely with practitioners, PCC staff, and clients to deliver high-quality, patient-centered care.

PCC / Administrative Responsibilities:

• Maintain patient records, ensuring accuracy and confidentiality.
• Manage practitioner relationships and coordinate scheduling needs.
• Respond to client requests via calls and emails in a timely, professional manner.
• Support providers by preparing patient documentation and managing appointment logistics.
• Facilitate communication between patients, providers, and internal teams.

Preferred Experience (Not Mandatory):

• Previous experience in patient care or clinical coordination.
• Familiarity with electronic medical records (EMR) systems.
• Experience working in a medical office or healthcare setting.

Qualifications:

• Active and unrestricted LPN license.
• Strong organizational skills with an eye for detail.
• Excellent communication and interpersonal skills.
• Ability to multitask and prioritize effectively in a fast-paced environment.
• Maintains a high level of professionalism and confidentiality.
• Proficiency with Google Docs, Sheets, and Gmail.
