Counselor - Outpatient

Crossroads Health provides a continuum of quality life-changing behavioral health and specialized chemical dependency treatment services to children, adolescents, young adults, adults and families. As a leader in the behavioral health field, we offer a vast array of programs and services that evaluate, educate, strengthen and support thousands of children, adults and their families each year.

We are currently seeking an empathic and passionate Counselor to provide individual and family therapy to our clients in our Mentor, Lake County office. The Outpatient Counselor offers diagnostic assessment and counseling within the office setting. This position is responsible for individual, family, and group services. This licensed provider will engage as a member of one or more collaborative teams. With a Trauma Informed approach, this position will be a representative for the organization and how we offer integrated services for recovery and mental health. Position will have adult clients and duties include:

  • Performs diagnostic assessments as necessary
  • Develops client-centered ISPs, monitor and review progress
  • Provides outpatient individual, group and family therapy to children, adolescents, and families
  • Maintains clinical records within Crossroads' standards
  • Arranges appointments and collateral contacts, and coordinates therapeutic efforts
  • Participates in outcome tracking
  • Provides direct service to clients in the event of an emergency, or in the absence of an assigned service provider

Requirements:

  • Bachelor's Degree with 3 years of experience. Master's Degree in Counseling or Social Work preferred.
  • LSW, LPC, MFT required; LISW, LPCC or IMFT preferred.
  • Two (2) years of experience in the mental health field (preferred)
  • Must have excellent interpersonal communication skills
  • Must be able to work on a team in a fast-paced, high stress environment
  • Must possess excellent crisis management/de-escalation skills
