QIDP/Social Worker– 40 hrs/wk (900QIDP)

Bayville, NY

Available Shift: Tuesday - Saturday, Flexible/varies

Salary: $55,000 – 60,000/yr

Qualifications:

  • Bachelor’s Degree in a health related field.
  • May have MSW from an accredited school of social work, NYS Social Worker Certification/Licensure.
  • Experience working with individuals with Developmental Disabilities preferred

Key Responsibilities include, but not limited to:

  • Work with individual residents and the IDT regarding scheduling, planning, implementing and supervising of all individuals' active treatment in the form of ADL/ILS goals.
  • Instruct and consult with direct care staff regarding their participation and roles in various ADL/ILS programming.
  • Complete observations of all staff implementing formal goals, ongoing services, and staff actions.
  • Provide assistance and training as needed to all staff when implementing individual treatment plans.
  • Act as a client coordinator.
  • Familiarize self with all ICF regulations including, but not limited to; 483, 633, 624, and life safety codes.
  • Identify the needs of the resident considering their family and life experiences.
  • Help resident's family develop constructive and meaningful ways to support the resident's experience in the residence.
  • Participate in interdisciplinary planning for the purpose of monitoring and follow-up of individualized program plans.
  • Develop channels of communication and consultation with direct care staff and professional staff to interpret and explain the resident's psychosocial needs as he/she progresses through his/her program plan.
  • When appropriate, explore other resident living alternatives and initiate a referral process.
  • Provide follow along services when a resident is discharged from residence to assure resident's adjustment in new setting.
  • Participate in in-service training.
  • Act as liaison between Day Program and residence to assure consistency for resident and eliminate duplication of service.
  • Assure requirements for audits are in place.
  • Monitor and assist the residents with their banking needs. Quarterly, balance resident bank accounts and submit to the Residence Director for inspection.
  • Annually complete a survey with the Residents and follow-up on any areas of concern.
  • Review and update, annually and as needed, the Comprehensive Functional Assessment, Abilities Profile, Face Sheet and Developmental Disabilities Profile.
  • Assure all evaluations are updated.
  • Participate in the internal audit process, as needed, but at least quarterly. Review deficiencies with IDT and provide a system for follow-up. Submit written report to the Director of Rehab Services.
  • Monitor and update Medicaid/Medicare/Social Security status of residents on your caseload
