Job Summary
The licensed Clinical Social Worker (LCSW) provides behavioral health support services to patients within the integrated care model at a Federally Qualified Health Clinic. This role involves assessments, short-term therapy, crisis intervention, care coordination, and collaboration with other providers and community partners to support patients behavioral health needs.

Responsibilities

• Conduct comprehensive assessments to determine client needs and develop individualized treatment plans.
• Provide therapy and counseling services to individuals, families, and groups dealing with mental health issues.
• Implement evidence-based practices, including motivational interviewing techniques and solution-focused therapy.
• Coordinate care and referrals with community organizations, mental health providers, and support services.
• Facilitate crisis intervention sessions to address immediate client needs.
• Document all clinical encounters and patient interactions in the electronic health record in a timely and accurate manner.
• Screen patients for mental health, substance abuse, trauma, and social determinants of health.
• Provide crisis assessment and intervention for patients presenting with urgent behavioral health needs.

Qualifications

Current LSW license in the state of Louisiana.

At least 5 years of experience in a clinical, community health, or primary care setting preferred.

Skills

• Strong background in social work principles and practices.
• Experience in addiction counseling and behavioral health interventions.
• Knowledge of childcare practices as they relate to family dynamics.
• Ability to manage crises effectively while providing compassionate care.
• Strong interpersonal, communication, and documentation skills.
• Excellent communication skills, both verbal and written, for effective client interaction.
• Strong organizational skills to manage multiple cases simultaneously.
