SUMMARY:
The LCSW/LCPC provides individual, family, couples and group counseling/therapy to help
patients attain changes that are beneficial to overall health. Provides consultation to medical
staff at all Alivio Medical Center sites.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Manage Behavior Health referrals from providers and school based health center
- Identify psychological, emotional and behavioral issues based on information obtained from mental health assessment
- Develop and implement individual treatment plans for patients based on information gathered in mental health assessment
- Provide consultation to medical providers and school based health center staff as well as staff at partnering schools
- Provide referral/case management to clients as needed
- Gather information about community needs or resources to help patients social/emotional needs
- Participates in patient staffing and care conferences, and in the development and implementation of multi-disciplinary approaches to patient care
- Consults with Center staff concerning psychosocial management of patient care;
- Identifies patient and family needs through case finding
- Interprets psychosocial information to the patient’s physician and other health care team members as appropriate
- Contacts and develops community resources on behalf of patients and the Center, and acts as patient advocate as appropriate
- Documents counseling services through recording in the patients medical record and completion of reports and statistics
- Assists in risk management though immediately alerting center Director of situations of high potential for injury, liability or legal action
- Participates as liaison between Alivio Medical Center, partnering organizations and schools, to provide educational groups for patients and/or families and cooperates with staff from other disciplines to plan and develop appropriate services for specific programs
- Maintains current knowledge of:
a. Changes in counseling/social work practices
b. Development in community resources
c. Regulations and laws affecting counseling services, programs and the health welfare of patients
- Represents Alivio Medical Center in professional and community organizations as assigned
- Participates in staff meetings and other meetings as assigned
- Trains and supervises students and volunteers as assigned
- Performs other job related duties as assigned
This position requires compliance with all of Alivio’s written standards, including its Standards of Conduct, Joint Commission standards, all policies and procedures and Corporate Compliance requirements. Compliance will be considered as part of the regular performance evaluation.

Supervise professional counselor and/or students to obtain appropriate licensure/and educational requirements

WORKING CONDITIONS:
OSHA Category 3 – Involves no regular exposure to blood, body fluids, or tissues, and tasks that involve exposure to blood, body fluids, or tissues are not a condition of employment
QUALIFICATIONS:
Education:
- Master’s Degree in Social Work, Counseling or related field from an accredited school. A concentration in clinical services is desired
Certification or Licensure:
- Current Illinois license as Licensed Clinical Social Worker (LCSW) or License Clinical Professional Counselor (LCPC)
Experience:
Demonstrated Competencies:
- Demonstrated acceptable skill level in psychosocial evaluations and treatment interventions. Experience in crisis intervention and brief psychotherapy is desired
- Demonstrated ability to develop and maintain good working relationships with patients, families and professional colleagues. Experience in interdisciplinary team approach to patient care is desired. Flexibility and ability to cope with stress. Adherence to professional ethics and standards consistent with the National Association of Social Workers, and Society for Clinical Social Work.
- Bilingual – English/Spanish a must; bicultural is preferred

Salary Range: $58,000.00-$65,000.00 Based on qualifications and experience.
