Social Services Assistant / Resident Care Coordinator
Position Summary:
The Social Services Assistant / Resident Care Coordinator plays a vital role in supporting the emotional, social, and transitional needs of our residents. Working closely with the Social Services Director and interdisciplinary teams, this role helps ensure high-quality care planning and resident advocacy throughout their stay. Position starting at $20/hr increased based on skills and experience
Key Responsibilities may Include (but are not limited to):
Resident Admissions & Assessments
Assist the Social Services Director with admission evaluations, including social history assessments, BIMS, and communication evaluations
Prepare rooms and welcome new residents upon arrival
Provide key information and support to families during the admission process
Care Planning & Coordination
Compile and analyze resident concerns, needs, and goals related to social services
Help implement individualized social service care plans
Participate in care conferences for new and long-term residents (quarterly or as needed), ensuring case managers and family/friends are invited and involved
Legal & Financial Support
Support residents in managing financial and legal matters, including navigating insurance and Medicaid/Medicare services
Provide guidance with Advanced Directives, Health Care Proxy, and Power of Attorney (POA) documents
Resident Advocacy & Psychosocial Support
Help ensure each resident’s emotional and social needs are supported and addressed consistently
Assist with PASRR screenings during admission and as needed
Assist in responding to and resolving resident and family grievances
Assist with Discharge Planning
Coordinate post-discharge services such as:
Home Health Services
PT/OT (Physical and Occupational Therapy)
In-home caregivers and related support
Team Support
Support the Social Services Director with documentation and daily resident follow-ups
Assist in maintaining compliance with regulatory guidelines and facility policies
Foster a compassionate and welcoming environment for residents, families, and staff
This role is essential for ensuring a smooth, supportive, and person-centered experience for residents—from admission to discharge.
Requirements:
- Long Term Care Experience Required
- Minimum of an Associate Degree
- Great customer service skills
- Passion to work with the aging population
- Great communications skills
