We are a mission-driven organization committed to supporting older adults, family caregivers, and veterans through person-centered care and access to community resources. We are seeking a dedicated and compassionate Care Manager to serve in our National Family Caregiver, Alzheimer's Respite Care, and Veterans programs, providing essential support to caregivers and their loved ones.
Job Summary:
The Care Manager is responsible for assessing client needs, developing individualized care plans, and coordinating services that promote independence and well-being. This role emphasizes caregiver support and advocacy within an atmosphere of respect, empathy, and collaboration.
This position Full Time- 35 hours a week.
Key Responsibilities:
- Serve as the primary contact for clients and their family caregivers.
- Conduct initial in-home assessments, reassessments, and develop care plans.
- Provide program information and explain care options to caregivers and clients.
- Coordinate service delivery and collaborate with all involved providers.
- Authorize service orders and monitor ongoing care needs.
- Avoid duplication of services and ensure timely discontinuation when appropriate.
- Maintain detailed documentation of care plans, progress notes, and client records.
- Advocate for clients and offer crisis intervention and referral support.
- Assist with documentation for CHCPE/CFC and other relevant programs.
- Build strong working relationships with hospitals, social workers, senior centers, and other stakeholders.
- Participate in outreach and education for caregiver and respite services.
- Report suspected abuse, neglect, or exploitation in accordance with DSS guidelines.
Qualifications:
- Bachelor’s degree in nursing, social work, public health, human services, or a related field.
- At least two years of experience in healthcare or human services.
- Strong skills in interviewing, assessment, and care planning.
- Knowledge of disabilities, family dynamics, and available community resources.
- Ability to evaluate, negotiate, and plan care across complex systems.
- Compassionate and person-centered approach to service delivery.
- Valid driver’s license and reliable transportation required.
- Proficiency with computers and ability to use a laptop in the field.
- Second language is a plus.
