Unity Place Partial Care - Cherry Hill, NJ

Partial Care Mental Health/Substance Abuse/Addictions Facility seeking compassionate Primary Care Coordinator who enjoys the challenge of working with Mental Health population.

Education:

• Bachelor’s Degree

Skills/Qualifications:

• 2-5 years of Human Services experience required
• Utilizing treatment team model
• Excellent documentation skills
• Responsibilities include treatment planning and all other required documentation
• Case Management, crisis intervention & facilitating psycho/education groups
• Provide exceptional individualized/group services
