About the position
- Provides direct social service and counseling to patients/families
- Assists in locating appropriate resources for post-discharge care
- Participate in multidisciplinary team to coordinate the client's plan
- Cross-trains with team members to serve as back up resource
- Assesses and identifies cases for intervention
- Prioritizes goals and develops plans based on client needs
- Monitors and evaluates outcome of client's plan
- Collaborates with third party payors internal case management/disease management departments as appropriate for discharge planning needs
Job Requirements
Requirements:
- Bachelors degree or Masters degree
- Current licensure by Texas State Board of Social Work
- LMSW, LCSW, or LPC preferred
- BLS issued by the American Heart Association
- 1 year social work experience
- acute care experience
