Are you a compassionate and detail-oriented professional with a passion for helping others? We are seeking a Hospice Intake Coordinator to join our dedicated team. In this vital role, you will be the first point of contact for patients and families exploring hospice care, guiding them through the admissions process with empathy, efficiency, and professionalism. As part of a supportive interdisciplinary team, you will help ensure a smooth and timely transition into care, making a meaningful impact during one of life’s most important journeys.

  • Competitive Pay – $18-$20 per hour depending on experience
  • Flexible Schedule with consistent full time hours: Monday-Friday 9A-5P. There will be some after-hours availability needed.

As an Intake Coordinator, you will:
  • Manage referral intake
  • Patient screenings
  • Insurance verification
  • DME orders
  • Scheduling of hospice admission visits

A few must-haves for an Admissions Coordinator:
  • Minimum of a High School Diploma or GED; an Associate’s or Bachelor’s degree is preferred
  • Preferably 2–3 years of hospice, home health, or medical intake experience
  • Familiarity with Medicare billing and hospice eligibility criteria
  • Excellent organizational and attention-to-detail abilities, including data entry and documentation tracking
  • Strong customer service and interpersonal communication skills, including engagement with physicians, facilities, referral sources, and families
  • Ability to handle stress and multitask, managing interruptions while prioritizing admissions and patient needs
