Serves as a member of a multi-disciplinary team to provide information, resources and choices for post-acute services to patients and families through assessments and care coordination efforts to effectively link them to the appropriate level of care to meet the patients emotional, medical, and psychosocial needs.
Essential Duties and Responsibilities:
- Complex discharge planning based on Continuity of Care assessment and anticipated post-acute care needs in direct consultation with patient, family, and multidisciplinary team which incorporates choice
- Provides counseling services to patients and their families to assist with adjustment to illness and/or disability with regards to the patients cultural, religious, and ethnic background and needs
- Provides consultation to the Health Care team to establish and implement appropriate treatment goals and discharge plans with regards to the patients cultural, religious, and ethnic background and needs
- Interacts with outside agencies on issues of suspected abuse/neglect/domestic violence/guardianship and other social/legal matters
- Uses knowledge of insurance benefits and coverage guidelines to maximize resources
- Delivers CMS regulatory notices within CMS established timeframes
- Educates staff, patients, and families about available community resources
- Promotes a clean and safe environment of care, utilizing the SAFE error prevention habits
- Provides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct; maintains licensure and abides by NASW standards
Required Qualifications:
Education and Training:
- Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) and Master of Social Work (MSW) required
- Must have current CPR.
Work Shift Details: Days, 2 Weekend shifts required in a 6-week schedule period.

The final pay rate offered may be more than the posted range based on several factors including but not limited to: licensure, certifications, work experience, education, knowledge, demonstrated abilities, internal equity, market data, and more.
