Role Overview
The Care Coordinator Manager (LICSW) plays a crucial role in guiding members toward achieving their best health through effective care management and coordination services. This position involves collaborating with members, their caregivers, and healthcare providers to set and reach personalized health goals. As a dedicated advocate, the Care Coordinator Manager ensures quality care while utilizing cost-effective resources, acting as the primary point of contact within the care management program.

$5,000.00 Sign On Bonus

Work Arrangement

  • This is a Hybrid position requiring on-site work 4 days a week.

  • Must reside in the DC Metro area.

Responsibilities

  • Conduct assessments with members through face-to-face meetings or phone consultations to evaluate their care management needs.

  • Complete detailed, person-centered assessments that cover physical health, mental health, social determinants of health, and supportive needs.

  • Coordinate services related to physical health, behavioral health, and social support.

  • Manage medications effectively, including regular reconciliation and promoting adherence.

  • Identify barriers to care coordination and develop appropriate care management interventions.

  • Create comprehensive care plans aimed at overcoming issues that hinder health achievement.

  • Set clear goals with defined priorities and deadlines, sharing these with members and their families when appropriate.

  • Schedule necessary follow-ups, make appropriate referrals, and document progress towards health goals and problem resolution.

  • Facilitate collaboration among the Care Coordinator, Community Health Navigator, members, their caregivers, primary care providers, specialists, and community organizations.

Education & Experience

3+ years of professional experience as a LICSW, particularly with pediatric populations facing complex medical and behavioral health challenges in acute care, home care, or community health settings.

Licensure

Current and unrestricted Licensed Independent Clinical Social Worker (LICSW) license and hold a Master's Degree in Social Work.

The salary range for this position is expected to be between $78,600.00 and $107,100.00 in the Washington, DC area. Pay is determined by various factors, including job-related skills, experience, education, certifications, and training.
